The registration process must be done in person at the practice. A registration form must be completed in full or, if you have your registration card from your previous doctor, this can be completed and brought to the practice. For all new adult patients, whether permanent or temporary, two forms of ID are required, one of which must be photographic (passport preferably) and the other must have your current Cheltenham address, preferably a utility bill. Please note: a card and paper counterpart driving licence are only acceptable as one form of proof.

New Born Registration: Please come into the surgery with baby with birth certificate to register.

You will be registered with a particular doctor but we consider patients registered at Yorkleigh to be free to see whichever doctor they choose although it does help if you try and see the same doctor most of the time.

The practice operates a pooled list system where patients are registered with the practice and not with a specific doctor. Should you wish to be seen by a particular doctor then you need to let the receptionist know and your wishes can be recorded on your electronic medical record.
